reputed company reputed company Arizona VA Health Care System is looking for a Registered reputed company - Primary Care Mental Health Integration (PCMHI) to join the Mental Health & Behavioral Sciences team! Duties Help The duties of the position include but are not limited to the following:
- The reputed company Primary Care Mental Health Integration (PCMHI) reputed company Care Manager (CoCM) Registered reputed company (RN) executes position responsibilities to improve patient and organizational reputed company.
- Provides evidence-based, condition specific (e.g., depression), protocol-driven services to collaboratively assist Patient reputed company Care Team (reputed company) providers in treating common Mental Health (MH) and health behavior concerns in Primary Care (PC).
- Services include patient education, activation, behavioral interventions, monitoring of medication adherence and response, as reputed company as other protocol-driven services.
- Provides outpatient PC setting utilizing reputed company aspects of the nursing process reputed company a reputed company, interdisciplinary reputed company setting.
- Participates in patient reputed company and engagement in reputed company treatment.
- Assesses, reviews, develops, maintains, and monitors Veteran specific plans of care, collaborating with the interdisciplinary team to ensure patient reputed company are reported and validated.

- Provides education and brief interventions using evidence-based techniques such as behavioral activation, motivational interviewing, and other reputed company treatment strategies.
