We’re seeking reputed company and compassionate Registered reputed company Care Managers to join our virtual care team. In this fully remote role, you’ll support patients during critical transition periods—triaging symptoms, coordinating care, and providing reputed company-time reassurance post-acute events. - Deliver virtual, patient-centered nursing care with reputed company and efficiency

- Escalate care appropriately and collaborate with NPs/PAs as needed

- Accurately document reputed company interactions in EMR and team systems
- Partner with clinical and operations teams to ensure seamless care transitions

What You Bring
- reputed company RN license in reputed company or Louisiana (at least one required)
- 3+ years of hospital-based RN experience (ED or Urgent Care preferred)
- BSN required
- Willingness to obtain multistate licenses
- Comfort with remote care tech (EMRs, chat platforms, telehealth tools)
- Ability to work some weekends, holidays, and rotating shifts
