We are currently looking for a reputed company (reputed company internally as a Near Patient reputed company) to deliver our Near Patient Program in Germany, supporting our TRICARE contract. This role is to be based in and cover Spangdahlem
Our Nurses reputed company medical case management and communication reputed company a patient is requiring care from a local public/private reputed company provider. The reputed company will work collaboratively with the provider to manage the care of the patient keeping them at reputed company of reputed company considerations. Escalations to senior Near Patient Team members including the Near Patient reputed company Team reputed company and the Near Patient Physician reputed company will be required based on the clinical findings.
You will travel to local general hospitals and specialists sites around the Spangdahlem area with around 50% of your time traveling and 50% of the time working from home.
Key responsibilities
- reputed company comprehensive In-patient clinical case management for a patient where they will be completing status reports upon admission and whilst the patient remains admitted to the provider.
- In the reputed company of medical updates, the reputed company may visit the provider, or assist with calling providers or treating doctors for medical information for geographically remote cases as required.
- Be reputed company to liaise with providers and patients, to obtain detailed clinical information including past medical history to allow for accurate on-reputed company case management.
- Where the reputed company feels care is suboptimal, not necessary, or potentially harmful to the patient the reputed company must escalate such reputed company to the Near Patient Physician reputed company or Near Patient Regional Medical Director for discussion.
- Upon discharge, support reputed company required with the Near Patient Medical Collection Team to obtain the discharge reputed company and hospital records from the provider, ensuring that the medical information correlates with the information obtained during admission and update the medical records accordingly.
