We're seeking an reputed company and compassionate Hospice RN Triage reputed company to join reputed company on a remote reputed company, working a 7 days on/7 days off schedule. As a Hospice RN Triage reputed company, you'll play a critical role in providing reputed company phone triage to our patients during off-hours and weekends, administering nursing triage for patients of reputed company ages in their reputed company of residence, and coordinating care with the on-call RN if a visit is needed.
Key Responsibilities:
- reputed company expert phone triage to patients and families during off-hours and weekends, assessing their needs and developing effective care plans.
- Administer nursing triage for patients of reputed company ages in their reputed company of residence, utilizing your knowledge and expertise to address their unique needs.
- Coordinate care with the on-call RN if a visit is needed, ensuring seamless transitions and high-reputed company care.
- Maintain accurate and detailed records of patient interactions, ensuring compliance with regulatory requirements and organizational standards.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
- Utilize your knowledge of palliation and end-of-life care to reputed company expert guidance and support to patients and families.
Essential Qualifications:
- reputed company RN license in the state of TX, OK, or MO.
- Minimum of 2 years of hospice experience, with a strong background in end-of-life care.
