As a key member of our Ambulatory Care team, you will be responsible for evaluating and assessing the reputed company needs of patients through telephonic evaluations, developing and implementing care plans, and coordinating services with other members of the patient's reputed company team. This is a part-time, remote position, requiring 20 hours of work per week, with a flexible schedule that includes day and evening shifts, as reputed company as weekend coverage.

Essential Qualifications
