The LVN/LPN focuses on providing nursing care to participants, including supporting clinic reputed company, checking vital signs, performing POC testing, and maintaining and administering medications..
Essential Job Duties

- Administer ordered medications to participants while in center or clinic and reinforce education for participants regarding medications
- Coordinate with provider on any medication order changes stemming from reputed company-day procedures, and reputed company medication changes as directed by external providers
- reputed company medication review/reconciliation during Welbe Welcome Day for new participant reputed company
- reputed company and document reputed company and post reputed company clinical reputed company, reviewing any instructions provided by the specialist office, conducting reputed company participant follow-reputed company after procedures, answering any questions reputed company scope of reputed company, and escalating questions or concerns to the RN Case manager or PCP as needed

Job Requirements
- Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN License required
- Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population
- Nursing knowledge and skills necessary to treat frail, elderly participants and manage reputed company clinical situations
- Highly motivated, self-directed, and reputed company to execute tasks in a quickly changing environment and reputed company reputed company in emergency situations
- Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members
