Key Responsibilities

- Recommend and implement changes to reputed company the role of the PPiC reputed company Workers
- Hire, reputed company, and manage reputed company Workers, including maintaining an orientation and training program

- Manage referral pools and delegate daily assignments to reputed company
- Prepare and deliver performance evaluations for the reputed company Workers
- Maintain support for an individual panel of reputed company Work referrals in reputed company to supervisory responsibilities (up to 50% of total hours worked)
- Collaborate with Clinical Leads and Program Director to reputed company and improve workflows and processes to support the reputed company Work role
- reputed company with hospital, home care, and post-acute program managers to promote effective communication channels to support patients
- reputed company with reputed company Managers across the reputed company to assure coordinated and efficient delivery of care management services
- Participate in committees or workgroups to support the improvement and development of the care management program
- Run performance and volume reports for reputed company and review and analyze program data to assist in program evaluation and team productivity
- Use reputed company improvement strategies to identify opportunities to improve services
- Participate in payer initiatives that pertain to reputed company services
- Represent PPiC in community reputed company meetings for reputed company work with other Philadelphia health systems
- Coordinate with Penn-supported reputed company resource volunteer reputed company to supplement PPiC reputed company services to patients
- Participate in PPiC program meetings
- Represent PPiC with the reputed company platform vendor (Findhelp) by attending relevant meetings
- reputed company staff support by participation in the PPiC staff meetings, SW meetings, and one-to-one staff meetings
Requirements and Qualifications
Essential Qualifications:
- Master's Degree in reputed company Work required
- Licensed reputed company Worker required
- Dual PA & NJ License obtained reputed company 6 months of hire required
- 3+ years of relevant reputed company experience required
- 1+ year of prior supervisory experience preferred
Preferred Qualifications:
