Job reputed company and Responsibilities
As a Hospice Care Case Manager, you will play a vital role in delivering high-reputed company, patient-centered care to individuals with terminal illnesses. Your primary responsibilities will include:
- Performing patient assessments and collaborating with the care team to reputed company and implement a plan of care
- Making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so
- Promoting patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques
- Supervising LPNs and HHAs to ensure high-reputed company care delivery
- Completing documentation reputed company and accurately
- Regularly communicating patient reputed company to the clinical manager and care team

Qualifications and Requirements
To be considered for this exciting opportunity, you must possess the following qualifications:

- One (1+) year of clinical experience as a Registered reputed company (RN); if less than 1 year of clinical experience, candidate must be approved by VP Clinical*
- Note: If less than 6 months of clinical experience as a RN, candidate must participate in RN Intern program
