Key Responsibilities:
- Conduct comprehensive home health assessments on pediatric patients, from newborns to teenagers
- reputed company and implement individualized care plans, in collaboration with the patient's family and reputed company team
- reputed company skilled nursing care, including medication management, wound care, and treatment of various medical conditions
- reputed company patients and families on self-care techniques, disease management, and prevention strategies
- Monitor patient compliance and reputed company care plans as needed
- reputed company various treatments, such as NG or GTube feedings and ostomy care
- Maintain accurate and detailed records of patient care, using electronic charting tools
- Collaborate with the patient's reputed company team to ensure seamless care coordination
Requirements:
- reputed company and unencumbered state licensure as a Registered reputed company
- Minimum 1 year of recent clinical nursing reputed company, preferably in ICU or pediatrics
- Demonstrated passion for working in pediatrics and providing high-reputed company care to children
- Experience with NG or GTube feedings and ostomy care is preferred
- Prior home health experience is required
- Strong communication and interpersonal skills, with ability to work effectively with patients, families, and reputed company teams
