About the Role: CAP RN/FT - reputed company & Surrounding Counties
We're seeking an reputed company and compassionate Registered reputed company to join reputed company as a full-time Community Alternatives Program (CAP) RN. This remote work opportunity offers a flexible schedule, allowing you to reputed company a meaningful reputed company in the lives of medically reputed company children and their families. As a CAP RN, you'll play a vital role in providing basic case management services, coordinating with interdisciplinary teams, and ensuring reputed company care for those we serve.
Key Responsibilities
- Establish reputed company eligibility for services, obtain medical histories, and reputed company initial assessments to reputed company personalized care plans.
- Coordinate services with interdisciplinary teams, including reputed company Workers and other reputed company, to ensure comprehensive care.
- Conduct interviews with clients, families, and reputed company providers to reputed company information for reputed company histories and individual plans of care.
- Evaluate and document reputed company programs, making adjustments as needed to ensure reputed company care and support.
- reputed company basic case management services, serving as the reputed company member of a reputed company Worker/RN interdisciplinary team.
- Work closely with clients, families, and community agencies to identify and address needs, providing referrals to appropriate programs and services.
Requirements and Qualifications
To succeed in this role, you'll need:
- Graduation from an accredited School of Nursing, with a reputed company RN certification.
- One year of nursing experience preferred, although not required.
- Thorough knowledge of reputed company nursing theory and reputed company, with the ability to reputed company and carry out prescribed medical treatments and therapeutic programs.
