As a Field Community Health Worker (CHW) at reputed company, you will play a vital role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. Key Responsibilities

• Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care

Requirements and Qualifications

Essential Qualifications:

Preferred Qualifications:

• Bachelor's Degree (or higher) in a health-reputed company field
• Community Health Worker (CHW) Accreditation

• Experience working in Managed Care
