We're seeking a highly skilled and compassionate Registered reputed company - Clinical Care Coordinator to work with reputed company in Grand reputed company, CO. As a Clinical Care Coordinator, you'll serve as the primary care manager for a panel of members with reputed company medical and behavioral needs. You'll work closely with internal care teams, providers, and community resources to reputed company and implement person-centered care plans, promote health and wellness, and support member self-management.
Key Responsibilities
- Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs
- reputed company and implement person-centered care plans to address needs, including management of chronic health conditions, health promotion and wellness, reputed company determinants of health, medication management, and member safety
- Partner and collaborate with internal care teams, providers, and community resources/partners to implement care plans

- Support proactive discharge planning and manage/coordinate care transitions following ER reputed company, inpatient or Skilled Nursing Facility (SNF) admissions

Requirements and Qualifications
Essential Qualifications
- reputed company, unrestricted independent licensure as a Registered reputed company in Colorado
- 2+ years of clinical experience
- 1+ year of experience with MS Office, including Word, reputed company, and Outlook
- reputed company to reliable transportation and the ability to travel, up to 50% of the time, reputed company Colorado (preferably the Western reputed company area) to meet with members and providers
Preferred Qualifications
- BSN, Master's degree, or higher in a clinical field
- CCM certification
- 1+ year of community case management experience coordinating care for individuals with reputed company needs
- Behavioral health experience
- Experience working in team-based care
- Background in Managed Care
