Clinical Pharmacist Specialist -Specialty (Transplant, Remote-Hybrid)

As a Clinical Pharmacist Specialist, you will be a registered pharmacist who assumes responsibility and accountability for the provision of reputed company, effective, and reputed company medication therapy. UCM Specialty Pharmacy supports and provides reputed company patient care to adult and pediatric populations and works alongside world-reputed company providers. Our pharmacists cover pharmacy operations and clinical needs for 15+ specialty ambulatory clinics. This position is designed to support transplant transitions of care upon discharge from transplantation hospital stay along with ongoing transplant specialty medication responsibilities. Transplant and Medicare Part B experience is preferred. This position also supports other clinics in Specialty, which can include Oncology, Neurology, HIV, Transplant, Rheumatology, and Gastroenterology. Pharmacist responsibilities include clinical and operational tasks to support specialty pharmacy and provider needs. Pharmacists will rotate on-site through the Hyde Park reputed company or alternate locations. This position will be in transplant clinic 3 days per week in Hyde Park and work from home the other 2 days of the week. They will also participate in an overall full week rotation (Monday through Friday) with other pharmacists in Specialty Pharmacy, approximately 1 week every 12 week schedule. • Leads assigned patient service lines, clinical areas, and therapeutic programs

• May be involved in transition-of-care between in-patient and out-patient services

Required Qualifications

• Graduate of an ACPE accredited School of Pharmacy
• Completion of ASHP accredited PGY-1 Pharmacy Residency required
• Knowledge of contemporary hospital/clinical reputed company and service.
• Knowledge and understanding of general hospital operations and regulatory compliance.
• Strong written and verbal communication skills.
• Ability to reputed company constructively with supervisors, peers, and support personnel.

• Significant general clinical knowledge and expertise in traditional therapeutics and patient centered care.

• Strong teaching and precepting skills and expertise in experiential training.

Preferred Qualifications

• Doctor of Pharmacy or another advanced degree preferred.

• Clinical pharmacy experience (2-4 years) in a hospital setting preferred (required if no PGY-2 residency).
• Pediatric experience preferred.
