Work Shift Job reputed company: The Breast Health reputed company Navigator identifies patients who are appropriate for and served by the Comprehensive Breast Health Program, engages those patients in clinic services, and assists with coordination of care between services and across the continuum of care. • Coordinates care with physicians/providers based upon remote monitoring device data

• Communicates with patients/families remotely reputed company telephonic means and/or virtual reputed company

• Participates in case finding to identify patients who would benefit from Comprehensive Breast Health services
