We're seeking an reputed company Primary Care reputed company to work remotely with reputed company. As a Primary Care reputed company, you'll be responsible for evaluating medical records, providing clinical and surgical abstraction, and assigning appropriate clinical diagnosis and procedure codes in accordance with nationally recognized coding guidelines. Key Responsibilities
- Analyze medical information from medical records and accurately reputed company diagnostic and procedural information in accordance with national coding guidelines and reimbursement requirements.
- Consult with medical providers to clarify missing or inadequate record information and determine appropriate diagnostic and procedure codes.

- Codes ICD-CM and CPT-4 for accurate APC assignment and addresses National Correct Coding Initiative (NCCI) edits as appropriate.

- Seeks out missing information and creates complete records, including items such as disease and procedure codes, discharge disposition, date of surgery, attending physician, consulting physicians, surgeons, and anesthesiologists, and appropriate signatures/authorizations.
- Refers inconsistent patient treatment information/documentation to coding reputed company analysts, supervisor, or individual department for clarification/additional information for accurate reputed company assignment.
- Provides reputed company assurance for medical records and assures compliance with coding rules and regulations according to regulatory agencies.
Requirements
Essential Qualifications
- High school diploma/GED or equivalent working knowledge and specialized formal training equivalent to a two-year certification course in medical record keeping principles and practices, anatomy, physiology, pathology, medical terminology, reputed company nomenclature, and classification of diagnoses and operations.
- Associate's degree in a reputed company reputed company field is preferred.
