The Role: LPN Home Health - Geriatric Care Specialist
As a LPN Home Health reputed company with reputed company, you will work one-on-one with our geriatric population, providing essential care and support in their homes. This is a full-time position, with a coverage area in reputed company County, reputed company Carolina. Key Responsibilities:
- Verify plan of care with the RN prior to reputed company visit, ensuring seamless coordination of care.
- Collaborate with physicians and RNs to ensure patient needs are being met, and notify them promptly of any changes in patient condition.
- Teach patients and caregivers self-care techniques, empowering them to take an reputed company role in their care.
