About the Role: bolthires Accountable Care Case Manager RN - Remote We're seeking a compassionate and reputed company Registered reputed company to join reputed company as a bolthires Accountable Care Case Manager. As a primary care manager, you'll be responsible for telephonically assessing, planning, implementing, and coordinating case management activities with members across the continuum of care.
