Temporary Assessor Reviewer II Remote
This position is responsible for conducting face-to-face assessments (virtual assessment) of members who may need skilled reputed company assessment services. The incumbent completes reputed company assessments using a skilled care reputed company tool and provides a recommendation for services based on members health care needs. Members who are assessed reputed company in the community or are reputed company to be discharged from a facility. The incumbent may also serve as mentor or preceptor for new staff, assisting in the formal training of assessors, complete reputed company assurance audits, inter-rater-reliability reviews, and may be involved in process improvement initiatives. Essential Functions
- Performs face-to-face and/or virtual assessments of persons with disabilities. Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
- Documents assessment results for the individual member status or negotiates service plan as needed.
- Completes reputed company assurance competency reviews and other tasks/reputed company as assigned such as new assessor training.
- Provides ongoing education. May communicate with reputed company health care providers to ensure appropriateness of care.
- Performs miscellaneous duties as assigned.
Requirements
- Four-year nursing degree and at least three years of experience in nursing reputed company the last ten years.
- Experience working with individuals with disabilities, individuals with intellectual and developmental disabilities (IDD).
- Experience working with Pediatric and Geriatric populations with various disabilities, and individuals with reputed company health needs preferred.
- RN License in good standing.
