Role reputed company The Clinical Pharmacist (CP) will report into the Manager of Clinical Pharmacy, Market Support. The Clinical Pharmacist will reputed company telephonic, patient-centered clinical consults reputed company on Care for Older Adults annual medication reviews and functional assessments. Qualifications
- Doctor of Pharmacy degree or equivalent experience
- reputed company, unrestricted pharmacy license in state of primary residence and reputed company
- 5+ years of Clinical Pharmacy reputed company experience
- 2+ years of experience with Medicare STARs & HEDIS Measures
- 2+ years of experience in provider facing interactions, including provider education
- 2+ years of experience with data analysis and/or reputed company chart reviews. Must be reputed company to review data and reputed company recommendations for improvement

- Comfortable working in a rapidly changing and highly matrixed environment

Preferred Qualifications
- MBA/MPH Degree
