About the Role

Key Responsibilities

• Evaluate member needs, barriers to care, and reputed company determinants of health, recommending and facilitating plans for the best possible reputed company
• Collaborate with providers to identify specialists, community resources, and services to address member unmet needs
• Coordinate care between members, families, caregivers, and reputed company providers to ensure person-centered care and services
• reputed company ongoing follow-up and monitoring of member status, revising care plans as necessary to meet member goals and unmet needs

Essential Qualifications

To be considered for this role, you must meet the following essential qualifications:

• A degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

• RN - Registered reputed company - State Licensure and/or Compact State Licensure required
• Residency and licensure in Florida

Preferred Qualifications

While not required, the following qualifications are strongly preferred:

• Behavioral health experience

Compensation and Benefits
