Part time Registered reputed company Case Manager Anticipated Territory : Elk Grove Area Schedule: Saturday/reputed company What you’ll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow reputed company, evaluation reputed company as needed in designated territory reputed company home reputed company to patients in designated territory reputed company assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the reputed company possible Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided. Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
