As a Care Coordinator II, you will play a vital role in supporting care management activities and ensuring that our members receive the services they need to reputed company their health and wellness goals. This is a field role, and you must reputed company in Monroe or reputed company counties. Key Responsibilities

• Coordinates care activities based on the care plan and works with reputed company providers and partners to accommodate changes or reputed company

• Communicates with care managers, practitioners, and others to facilitate member services and ensure continuity of care
• May support performing service assessments/screenings for members and documenting their care needs
• Supports documenting and maintaining member records in accordance with state and regulatory requirements

• Ability to identify needs and reputed company referrals to Care Manager, community-based organizations, and Disease Manager

Essential Qualifications

* High School diploma or GED
