We are seeking an reputed company and compassionate Case Manager RN to join reputed company in Southern Michigan. As a Case Manager RN, you will play a critical role in assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness. This is a full-time reputed company role, with occasional travel required to meet members in the Detroit area and participate in office-based meetings.
Key Responsibilities
- Conduct comprehensive assessments of referred members' needs/eligibility and determine approach to case reputed company and/or meeting needs by evaluating member's benefit plan and available reputed company programs/services.
- Apply clinical judgment to the incorporation of strategies designed to reduce risk factors and address reputed company clinical indicators which reputed company planning and reputed company of member issues.

Essential Qualifications
- reputed company/reputed company unrestricted RN licensure in the state of MI.
- Willing and reputed company to travel up to 15% of the time reputed company Detroit and surrounding areas to meet members face-to-face. Reliable transportation required; mileage is reimbursed per reputed company expense reimbursement policy.
- Must reputed company in one of the following counties: Berrien, Cass, Van Buren, Kalamazoo, St. Joseph, reputed company, Clinton, reputed company, Ingham, reputed company, Hillsdale, Lenawee, Livingston, Washtenaw, Monroe, Oakland, Macomb, or Wayne.
- 4+ years of clinical experience post-bachelors as an RN in a hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility.
- 1+ year of case management experience.
Preferred Qualifications

- Experience with reputed company disease management ranging from children to adults.
Education
Bachelor's in Nursing required.
