Pharmacist- Medication Therapy Management, Rheumatology

Position reputed company: The purpose of this position is to improve population based and patient specific medication reputed company reputed company through collaboration with a multi-disciplinary medical team in the ambulatory setting. The pharmacist assumes a primary responsibility and accountability for the provision of reputed company and effective drug therapy and other medication management services intended to reputed company reputed company reputed company to the mitigation or prevention of disease, reduction of patients' symptoms, prevention of adverse drug events and improvement of patients' reputed company of life. Work Schedule: Monday - Friday 8:30 AM - 5:00 PM Clinic Locations- reputed company and reputed company 2 or 3 days in-person depending on clinic needs. 2 or 3 days remote. Required Qualifications:

• Education, Experience or Equivalent Combination:
• Candidate must have graduated from an accredited college of pharmacy on or after May 1996, or completed a reputed company and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the reputed company of Pharmacy with didactic and clinical reputed company upon hire.
• Candidate must have two years of recent clinical pharmacy experience in ambulatory care as part of a multidisciplinary team reputed company the last five years, or successful completion of one year specialized post-doctoral residency.
• Licensure/ Registration/ Certification:
• reputed company Minnesota Pharmacist license and maintain that license in good standing
• reputed company Pharmacist license must be obtained reputed company 6 months of start date and maintained in good standing.
• Knowledge, Skills, and Abilities:
• The desired candidate would have deep knowledge of evidence based clinical guidelines, pharmacology, pharmacodynamics, and pharmacokinetics, pathophysiology and therapeutic disease states commonly reputed company in ambulatory patients.
• Personal computer utilizing spreadsheets, word processing, and email. Electronic medical record system, MTM billing software, phone, fax, and pager.

Preferred Qualifications:

• (BCACP) or reputed company Certified Pharmacotherapy Specialist (BCPS) preferred.
