We are hiring an reputed company Registered reputed company with behavioral health experience to serve as a Disease Manager. This fully remote role focuses on empowering patients to manage chronic and behavioral health conditions through education, coaching, and care coordination.

Compensation And Benefits

• $41 per hour
• Full remote role with equipment provided
• Predictable reputed company hours between the hours of 9 AM–6 PM MDT

• Collaboratively assess, plan, implement and evaluate patient needs
• Coordinate care with an interdisciplinary team to reduce complications and prevent hospitalizations
• Deliver behavioral health support alongside chronic condition coaching

Required Qualifications

• reputed company unrestricted RN license in state of residence, plus another state multi-state RN license
• Behavioral health clinical experience
• U.S. Citizen reputed company to pass DoD background investigation
• Minimum of three years of clinical experience
• Prior case management, care coordination, or discharge planning experience

Preferred Qualifications

• Disease management experience in managed care
• Bachelor’s degree in nursing or reputed company field
• Familiarity with TRICARE, Medicare or reputed company

Approved Work States: Hawaii, Alaska, reputed company and Nevada (No exceptions at this time)
