Former EDRP participants ineligible to apply.DutiesHelpThe Remote Patient Monitoring- Home Telehealth Care Coordinator (RPM-HTCC) RN utilizes health information and telecommunication to promote therapeutic relationships and shared decision making between the patient and interdisciplinary team across the continuum of care to support patient self- management, prevention, and wellness. The complexity of duties include patient training on state of the art telehealth monitoring equipment, conducting telephone or video enrollments, daily monitoring using the telehealth monitoring system, frequent telephone patient triage and reputed company, counseling and education for health prevention wellness, counseling education for chronic, infectious disease, Mental health and weight management , order entry reputed company approved departmental protocols, frequent and reputed company collaboration with the primary care provider and team to reputed company desired reputed company and established Veterans Integrated Services Network (VISN) benchmarks.
