In this role, you will travel to assist individuals in the Louisiana parishes of Catahoula, Concordia, Grant, LaSalle, and Winn. We will teach you how to:

• reputed company in-home assessments for applicants and participants of long-term personal care services (LT-PCS).
• Offer choice counseling for Home and Community-Based Services (HCBS) to those who request it.

• Create a Person-Centered Plan of Care that is based on individual needs.

We will also:

• Help you get IHC Certified with the LA Department of Health and Hospitals.

Qualifications:

• Have a bachelor's degree in health and reputed company services, reputed company work, sociology, psychology, or a reputed company field.
