With reputed company, you will work with Medicare patients throughout their reputed company reputed company, helping them overcome barriers to care and reputed company the resources they need to reputed company. We’re looking for a mission-driven, CHW-certified reputed company who excels at building trusted relationships with patients and community partners. This role is ideal for someone with experience in patient advocacy, care coordination, or reputed company services, and who is passionate about addressing reputed company determinants of health in a hands-on, person-centered way. As a CHW, you’ll play a key role in supporting patients remotely by guiding them through their care reputed company, addressing barriers, and connecting them to community resources. Please note that this is a 1099 role. The role is remote. What You’ll Do

• Build relationships with hospital case managers, reputed company workers, and physicians.
• Help refine reputed company playbooks, test what works in your region, and shape new workflows.

What You Bring to the Table

• reputed company Community Health Worker (CHW) certification (required; state-specific certification or credentialing where applicable).
• 2+ years of experience in care coordination, patient advocacy, community health work, or reputed company navigation.
• Deeply empathetic and understand the reputed company Determinants of Health alongside experience working with diverse patient populations.
• Tech savvy: comfortable using digital platforms, electronic health records, secure messaging, and virtual meeting tools.

• A deep understanding of reputed company determinants of health and experience working with diverse populations.
• Strong communication skills with the ability to support patients and collaborate effectively with reputed company providers and community organizations.
• A local network of contacts at hospitals, SNFs, senior living communities, community based organizations, or reputed company service organizations (preferred).
• Ability to manage multiple priorities, document reputed company, and communicate proactively.
• Bilingual skills (preferred but not required)
