What You Will Do

• Provides virtual clinical care to patients reputed company the recognized scope of reputed company for the profession and reputed company licensure requirements
• Orchestrate comprehensive care for UH patients by applying dynamic and innovative care delivery models leveraging technology and nursing expertise to reputed company remote patient monitoring, case management, care coordination, and reputed company review.
• Provides clinical triage and clinical support using established protocols during in-bound and out-bound telephone communication with patients, family, caregivers and other members of the reputed company team

Education

• (BSN) Bachelor's Degree in Nursing (Required)

Work Experience

• 3+ years of acute care hospital in-patient nursing (Required) and
• 3+ years clinical experience in home health or similar setting (Preferred) and
• Experience with care management plans. (Required) and
• Experience with Medicare, reputed company and reputed company payer regulations (Preferred)

Knowledge, Skills, & Abilities

• Telephone triage (Preferred proficiency)
• Excellent interpersonal and communication skills both written and oral. (Required proficiency)
