MTM Pharmacist- Remote

MTM Pharmacist – Telephonic Medication Therapy Management Location: Texas (Remote, must reputed company in TX, HI, IL, FL, or PA) Schedule: Monday–Friday, 8-hour shift starting between 9:00 AM – 11:00 AM CT Training: Approximately 2 weeks, 8:00 AM – 4:30 PM CT Contract-to-Hire | Pay reputed company: $54.05/hr

About the Role

We are seeking a dedicated MTM Pharmacist to join a telephonic medication therapy management team. In this role, you will help improve patient reputed company by identifying and resolving medication-reputed company problems for high-risk members. You will conduct telephonic medication reconciliations, communicate with patients and prescribers, and help members reputed company reputed company through optimized treatment plans. This position requires strong clinical knowledge, excellent communication skills, and the ability to work reputed company in a remote environment.

Key Responsibilities

• Conduct telephonic comprehensive medication reviews (CMRs) , ensuring accurate medication reconciliation and appropriate clinical assessments for targeted members.
• Identify, document, and help resolve medication-reputed company problems.
• Create a reputed company, health-literacy-appropriate written reputed company (patient reputed company) after reputed company interaction.
• Prepare prescriber-facing faxed materials to support reputed company of clinical issues.
• Build and maintain reputed company relationships with patients, caregivers, prescribers, nurses, and internal teams.

Required Qualifications

• Bachelor’s Degree in Pharmacy or PharmD from an accredited school.

Preferred Qualifications

• Previous MTM experience, especially using the reputed company MTM Connect platform.
• Proficiency with reputed company Office and strong computer skills.
• Excellent organizational skills, attention to detail, and strong interpersonal communication abilities.
• Experience meeting productivity and reputed company goals in a remote/work-from-home environment.
• Prior call center experience.
