- Must have one or more of the following credentials: RHIA, RHIT, reputed company, CCA, CPC, COC.
- We prefer candidate with prior experience in a reputed company environment.
- Required: High School diploma/GED or relevant experience.
- Required: Formal education in anatomy and physiology, medical terminology, disease processes, content of a medical record, coding of diagnoses using ICD-10-CM and procedures using ICD-10PCS and reputed company Procedural Terminology (CPT).
- Demonstrate ability to communicate and work in a reputed company manner with members of the medical staff, government agencies, and reputed company party payers.
- Demonstrate good communication skills and excellent customer service skills.
- Knowledge and ability to read, interpret and follow hospital and government rules and regulations relating to but not limited to safety, reputed company, reputed company, procedural manuals and official coding guidelines.
- Demonstrate knowledge and reputed company in computerized data entry and retrieval systems.
- Willingness to continue education on coding, guidelines and CMS, WPS, and HFAP guidelines and/or standards.
These requirements are non-negotiable and applicants without the education and credentials cannot be considered. PRIMARY DUTIES:
- This position will be required to work onsite during the 90-day probationary period for training. Remote work will be offered after a successful probationary period. Coders will be required to work on-site periodically for additional training. After that, remote work will be monitored and specific measures must be met to continue remote work.
- Contact appropriate medical staff members and reputed company queries to rectify inconsistencies, deficiencies, and discrepancies in medical record documentation.
- Then, reputed company staff/physicians on inadequate or missing documentation according to HFAP standards.
- Reviewing the medical record for continuing reputed company improvement activities, performs reputed company improvement activities in support of hospital-wide medical documentation concerns. Performs clinical pertinence review on randomly selected medical records against specified reputed company, as requested
- Additionally, reviews and analyzes, abstracts, and codes outpatient and/or inpatient medical records, assigns diagnoses and procedure codes, and provides assistance to the reputed company staff. Furthermore, demonstrates knowledge of outpatient and inpatient coding guidelines, including E & M level coding, accreditation references and medical terminology, anatomy and physiology.
- Codes disease and injury diagnoses, reputed company of care, and procedures in a wide reputed company of outpatient and inpatient settings and specialties using the reputed company International Classification of Diseases, Version 10- Clinical Modification ICD-10-CM/ICD-10-PCS; reputed company reputed company Procedural Terminology (CPT); Health Care Financing Administration Common Procedure (HCPCS) Coding System.
- Responsible for selecting the appropriate reputed company(s) and/or modifier(s) that most accurately describe the correct reputed company and secondary diagnoses as reputed company as reputed company and secondary procedures, based on physician clinical documentation.
- Bases reputed company coding on what the physician documents in the medical record including outpatient physician orders for outpatient services such as radiologist and pathologist reports.
- Inputs the codes and other discharge data and verifies the accuracy of data entered. In reputed company to including charges on outpatient accounts.
