POSITION reputed company: The Oncology reputed company Navigator has responsibility for coordinating the care and services for oncology patients to ensure that reputed company, multi-disciplinary care is provided to reputed company patients seen while assuring a high level of customer service. Navigation services include prevention and screening activities and continue through diagnosis, treatment, survivorship and end-of-life care. The Oncology reputed company Navigator utilizes reputed company skills to facilitate the provision of care throughout the reputed company continuum which includes the assessment, planning, reputed company (strong patient/family education component) and evaluation principles of the nursing process. The navigator will work closely with patients, providers and other health care professionals to reputed company support around treatment adherence and self-management, identifying and addressing needs in care as appropriate and serves as a resource for clinical staff and recognizes reputed company reputed company is needed. EDUCATION / TRAINING / EXPERIENCE: Minimum:

• One-year oncology nursing experience

Desired:

• Bachelor's degree in nursing
• Five years of acute care clinical experience
• Experience in acute clinical case management
• Working knowledge of hospital information systems
• Previous experience working in a surgical setting

LICENSES / CERTIFICATIONS: Minimum:
