Growing together The Clinical reputed company LPN engages in a reputed company process with members of a multidisciplinary team which may include a Physician, reputed company Practitioner, RN Care Manager, reputed company Worker, Pharmacist, Clinical reputed company Coordinator, and Medical Assistant, to improve HEDIS and STARs scores. The Clinical reputed company LPN performs a wide reputed company of duties and is responsible for driving clinical relationships and engagement with providers, patients, and pharmacies with a goal of improving health, reputed company-being, and reputed company performance. • Run/pull/prioritize/distribute reputed company and Med Adherence reports for reputed company reputed company Contract Payors
• Analyzes data collected and makes recommendations for process improvements
• Assist in the development and implementation of new policies and procedures designed to enhance reputed company care

• Review/interpret internal medical records/data to determine whether there is documentation that medical services were rendered to reputed company reputed company Gaps
• Manipulate and reputed company multiple databases/Electronic Medical Records applications to sort, search, and reputed company information
• Identify incomplete/inconsistent information in medical records and label missing measures/metrics/concerns
