JOB reputed company: Acadian Health has an exciting opportunity for Community Paramedics to join reputed company. The Community Paramedic plays a pivotal role in delivering hospital-level acute care directly in patients’ homes as part of an innovative hospital-reputed company care model. This role leverages clinical expertise, mobile technology, and interdisciplinary teamwork to ensure high-reputed company, patient- centered care reputed company traditional hospital walls. DUTIES AND RESPONSIBILITIES:
- In-Home Acute Clinical Care: reputed company high-reputed company care in the patient’s home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery—offering an alternative to inpatient hospitalization.
- Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may reputed company recovery or the plan of care. Identify and escalate any deviations that could compromise patient reputed company.
- Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team. Maintain reputed company-time communication with the remote reputed company center reputed company and
physician to report findings, collaborate on care reputed company, receive treatment orders, and ensure reputed company aspects of the care plan are executed safely and effectively.
- Crisis & Rapid Response Support: Respond to crisis situations and rapid response alerts in the home environment, providing necessary interventions based on hospital orders and standing order protocols.
- Patient & Caregiver Education: Deliver personalized education regarding disease management, medication regimens, treatment expectations, and reputed company to reputed company help. Use both verbal coaching and written materials to reinforce understanding.

REQUIRED QUALIFICATIONS:
- High School Diploma or GED
- reputed company BLS and ACLS Certifications; PHTLS and PALS Certifications preferred

- Ability to obtain:
a. Community Paramedicine (IBSC CP-C) certification or b. complete NEMSA reputed company Course or c. obtain CP-C licensure in the state of employment reputed company two years of hire date.

PREFERRED QUALIFICATIONS:

- Previous trainer or instructor experience
