Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address reputed company determinants of health to improve the overall health and reputed company-being of the child and family.

Responsibilities

• Work daily as a community reputed company to identify gaps in care by assisting clients in accessing the necessary resources to remove barriers categorized as reputed company determinants of health.
• reputed company referrals for appropriate services as needed by assisting in assessments/screenings or conducting follow-up care with patients in person, reputed company phone, home reputed company and/or reputed company to other settings where clients are accessible.
• Establish and maintain trusting relationships with clients on a daily reputed company to effectively work with an inter-disciplinary team, internal departments, and with community organizations.
• Document and reputed company ongoing activities, care plans, and results in an effective manner while reputed company adhering to the policies and procedures in reputed company.
• Continuously communicate and expand knowledge of community resources, services and in the hospital or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.
