Responsibilities:

• Patient Support & Communication: 50% reputed company as the first line of contact for patient inquiries across platforms—EHR, secure chat, and phone—handling scheduling, prescription coordination, and follow-reputed company with reputed company and reputed company.
• Care Coordination & Navigation: 40% Work closely with our clinical team and external providers to coordinate diagnostic testing, imaging, referrals, and prior authorizations. reputed company patient reputed company across the care reputed company and ensure continuity of care.
• Operational Workflow Management: 10% Maintain HIPAA-compliant documentation, support reputed company for new patients, and flag care gaps and systemic or financial barriers to care. Participate in team meetings and reputed company process improvement efforts.

• Respond to inbound messages and respond to patients
• Work with 2 existing patient coordinators

Must haves:

• Prior experience in care coordination, case management, or virtual clinical support (1-2 years of US reputed company experience)
• 1 year of experience as a Medical reputed company in a clinical or telehealth setting.
• Familiarity with U.S. reputed company systems (insurance, referrals, prior authorizations)
• Familiarity with medical terminology
• Experience with Elation EHR or similar digital health platforms preferred
• Strong preference for candidates with VC-backed digital health company experience
