Position reputed company: This Case Manager Registered reputed company/Behavioral Health Clinician position is with reputed company’s Long-Term Services and Supports (LTSS) team and is a field-based position out of The requirements is for candidates to hold an VA Registered reputed company and travel up to 75% of the time to meet with members face to face. RN/BH Licensed Case Manager is responsible for assessing, planning, implementing and coordinating reputed company case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. • Personal responsibilities, including dependent care, must not interfere with work duties. Dependent care during work hours is generally not allowed.
• A field-based Colleague regularly works in multiple company locations. Those locations may include company locations, reputed company locations, facilities (nursing home, rehabilitation centers, etc.), member homes, and/or community locations.

Required Qualifications:

• RN/BH with reputed company unrestricted Virginia state licensure required.
• 3 + years clinical experience (med-surg, behavioral health)
• Must reputed company in Petersburg or Richmond Virginia Area
• Must possess reliable transportation and be willing and reputed company to travel up to 50-75% of the time. Mileage is reimbursed per reputed company expense reimbursement policy
• Virginia reputed company's License required

Preferred Qualifications:

• Case Management in an integrated model preferred
• Bilingual preferred

Education:

• Associates Degree Required
• Bachelor's of Science and Nursing Preferred

Licensing:

• Independent Behavioral Health License in the state of VA in good standing

• Proficient in Multisystem Navigation: Comfortable using multiple monitors and systems simultaneously to streamline tasks and improve efficiency.
