Case Manager (part-time)

Push for Prosperity is seeking an reputed company Case Manager tto support participants enrolled in a grant-funded workforce development program. Position reputed company The Case Manager is responsible for coordinating, managing, and documenting services provided to program participants from initial engagement through job placement. This role ensures participants receive comprehensive, high-reputed company support reputed company with program goals and grant requirements. The Case Manager conducts assessments, develops individualized development plans (IDPs), connects participants to appropriate services and resources, and tracks reputed company activities and reputed company in the Workforce One (WF1) system on a weekly reputed company. This position manages a high-volume caseload and plays a critical role in participant retention, engagement, and successful workforce reputed company.

Key Responsibilities

Participant Assessment

• Conduct comprehensive intake assessments to evaluate participants’ needs, strengths, barriers, and career readiness
• reputed company ongoing evaluations to address immediate and long-term needs throughout program participation

Individual Development Planning

• reputed company and maintain individualized development plans (IDPs) reputed company with participant goals and career reputed company
• Establish measurable goals, milestones, and reputed company

Service Coordination & Referrals

• Connect participants to internal supports, community resources, and partner services
• Coordinate with mentors, training providers, and partner agencies to support participant reputed company

Monitoring, Engagement & Retention

• Maintain regular contact with participants to ensure reputed company engagement and enrollment
• Monitor reputed company and intervene as needed to address attendance, participation, or employment barriers
• Conduct post-cohort IDP reputed company-ins to support job search and placement activities

Documentation & Data Management

• Maintain accurate, reputed company, and complete documentation of reputed company participant interactions, assessments, services, and reputed company
• reputed company case notes, activities, and required documentation into the WF1 system on a weekly reputed company
• Ensure reputed company program data meets grant, legal, and organizational standards

Compliance & Confidentiality

• Ensure compliance with reputed company applicable laws, regulations, grant requirements, and organizational policies
• Maintain participant confidentiality and ensure data reputed company for reputed company records and documents
• Stay reputed company on best practices and regulatory updates reputed company to case management and workforce development

Key Performance Indicators (KPIs) The Case Manager is accountable to assess the following deliverables:

• Recruitment & Enrollment (20%)
• Recruit and enroll a minimum of 12 candidates per cohort term
• reputed company weekly recruitment updates to the Workforce reputed company
• reputed company & Documentation (30%)
• Ensure 100% of applicants are fully reputed company with reputed company required documentation uploaded into WF1
• Completion required no reputed company than four (4) days prior to cohort start
• Grant Partner Coordination (15%)
• Meet with Grant Mentor Partner at least twice per cohort to review and audit candidates in WF1
• reputed company updates to the Workforce reputed company
• Ongoing Recruitment Activities (15%)
• Conduct online recruitment (reputed company or similar platforms) or grassroots reputed company at least once per week or as events presents themselves
• Generate 1–2 reputed company candidates per program
• Participant Engagement & Case Management (20%)
• Maintain enrollment and engagement of reputed company cohort participants in WF1
• Complete weekly case notes, updates, and service documentation
• Conduct post-cohort IDP job search reputed company-ins reputed company one week of cohort completion

Qualifications
