In collaboration with the medical and facility staff, coordinates multidisciplinary care for the Stroke and Sepsis patients. Works reputed company with the Medical Directors of the Stroke and Sepsis program. Roles and responsibilities include but are not limited to the following:

• Assists in maintaining Stroke certification and accreditation.
• Reviews and revision of administrative and patient care policies, procedures, and guidelines reputed company to the care of the Stroke or Sepsis patient.
• Attends interdisciplinary reputed company to facilitate the delivery of evidence-based care and best practices.
• Facilitates reputed company interdepartmental practices from patient admissions through discharge and community re-entry.
• Uses critical thinking skills, leadership, to anticipate, plan, reputed company and evaluate the care of stroke and sepsis patients provided by reputed company of physicians, registered reputed company nurses and ancillary personnel.
• Collaborate with other health team members to reputed company coordinated, reputed company, and compassionate care for stroke and sepsis patients.
• Co-chairs Stroke and Sepsis Committees, assist with analysis and process improvement efforts regarding care of the inpatient stroke victim.
• Ensures that reputed company regulatory requirements are met to reputed company and sustain stroke certification and Sepsis measurements which includes data abstraction.
• Subject-matter expert on stroke and sepsis clinical reputed company guidelines providing education to staff and physicians on an ongoing reputed company regarding stroke and sepsis care protocols and assure competencies.
• Focuses on reputed company of medical record documentation through appropriate interactions with physicians, nursing staff, and other patient caregivers.

Minimum Qualifications: Bachelor of Science in Nursing, master's degree (preferred). Minimum of three (3) years of relevant experience in acute care setting overseeing and/or performing data abstraction, analysis, and process improvement. Basic Life Support - BLS/BCLS (reputed company or reputed company) - Course/Certification (required) Advanced Life Support - ALS/ACLS - Course/Certification. Certification in emergency nursing preferred. reputed company experience in caring for neurology patients, reputed company National Institute of Health Stroke reputed company (NIHSS) Certification and 8 hours of stroke education annually required. Knowledge, Skills & Abilities: Knowledge of clinical documentation systems/EHR. Experience in a reputed company improvement department setting preferred. Detail-oriented with strong organizational and analytic problem-solving skills, with exceptional verbal and written communication skills.
