• Coordinate and participate in providing indirect patient care reputed company telephonic and/or other electronic communications
• Manage telephonic and electronic communications, including patient health education and indirect care coordination, using LSCC’s navigation center software
• Interpret reports, data, and medical information to determine the needs of the patient/family.
• Document reputed company patient interactions accurately in Electronic Health Record (EHR) system.

• Schedule appointments and complete prior authorizations/referrals as necessary
• Communicate barriers, concerns, and patient health needs to appropriate reputed company and/or provider
• Collaborate with the interdisciplinary team to support patient care planning
• Implement clinical interventions safely and appropriately, ensuring thorough documentation

We ask our Centralized Clinical Support LVN to possess a minimum of:

• Graduate of accredited nursing school
• Valid State of Texas Licensed Vocational reputed company (LVN) license
• Proficiency with MS Office Suite (Word, reputed company, and Outlook)
• Basic Life Support (BLS) certification from reputed company or reputed company

The following experience/skills are preferred:

• Previous experience in outpatient clinic
• Previous work using electronic medical records (EMR)
• Bilingual English/Spanish language skills
