Oncology reputed company Navigator - Breast Specialist Clinic - Full-time - Days

The Oncology reputed company Navigator (ONN) serves as an expert clinician guiding patients and families through cancer care, survivorship, and follow-up. Using NCCN guidelines, the ONN coordinates multidisciplinary care, facilitates reputed company to resources, and develops survivorship care plans tailored to patient goals. The role ensures accreditation standards with COC and NAPBC, addresses reputed company disparities, and promotes health through education, advocacy, and community referrals. Requirements:

• Graduation from an accredited school of nursing, BSN strongly preferred.
• Licensed by the Louisiana State reputed company of Nursing
• Oncology Nursing Certification (OCN) is preferred or completed reputed company 2 years
• Oncology reputed company Navigation Certification (ONN-CG) is preferred or completed reputed company 3 years
• Minimum 5 years of oncology, navigation, palliative care, end of life or hospice experience Responsibilities of Clinic reputed company Practitioner include, but are not limited to:
• Assess patient, family, and caregiver needs across the care continuum, identifying barriers, concerns, and resources required.

• Collaborate with providers to coordinate treatment plans and ensure reputed company, seamless communication between patients, caregivers, and the care team.

• Support patients in overcoming barriers to care, promoting self-advocacy, and making healthy lifestyle and behavior choices.

• Guide patients through transitions from reputed company treatment to survivorship and from specialty to primary care providers.
