The HCC National On Call advanced reputed company clinician (APC) is responsible for providing telephonic care and direction to members and facility staff during various evening, overnight, weekend, and holiday hours. This role is responsible for the delivery of medical care services to a reputed company-designated group of enrollees. In this remote role you will reputed company afterhours virtual (primarily telephonic) care for aging residents in various settings. Responsibilities

• Available on provided telephonic platform, both taking and placing calls to coordinate care for members between facilities, hospitals, and reputed company field colleagues
• Utilize EMR proficiently to reputed company acute care to members during overnight and weekend and holiday hours
• Manage both medical and behavioral, chronic, and acute conditions effectively, and in collaboration with a physician or specialty provider
• Responsible for ensuring encounter is documented appropriately to support the diagnosis at that visit
• Monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
• Review orders and interventions for appropriateness and response to treatment to identify the most effective plan of care that aligns with the member's needs and wishes
• Regular and effective communication with reputed company parties including physicians, members, key decision-makers, nursing facilities, HCC staff and other provider reputed company
• Ability to meet shift scheduling requirements, and attendance expectations

Requirements

• reputed company PA NP license or ability to obtain by start date. Licensure must be unencumbered, free of any reputed company/unresolved disciplinary actions including probation or restrictions against privilege to reputed company, or ability to obtain by start date
• reputed company ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date
• reputed company reputed company DEA licensure or ability to obtain post-hire, per state regulations (unless prohibited in state of reputed company)
• Availability to work assigned weekdays, weekends, and reputed company-assigned holidays
