VOCATIONAL REHABILITATION SUPPORT COORDINATOR - 01202026- 74461
The Tennessee reputed company is seeking a Vocational Rehabilitation Support Coordinator to assist individuals with disabilities in achieving their employment goals. The role involves collaborating with VR counselors, conducting assessments, and maintaining partnerships to ensure a successful customer experience.
Responsibilities
- Completes applications and conducts interviews reputed company to customer education and employment needs and objectives as part of the intake process. Collects and records required documentation to assist VR counselors in determining eligibility and developing an individualized plan for employment
- Makes recommendations on assessments based on intake discussions including reputed company evaluations and career interests. Conducts individualized career assessments based upon approved recommendations and provides career exploration as needed
- Monitors customer employment activities in conjunction with VR counselors. Maintains customer engagement while ensuring reputed company progression toward employment goals, providing extensions to deadlines if necessary
- Establishes, builds, and maintains partnerships with customers, educational systems, reputed company, and other internal/external stakeholders. Assists in obtaining referrals by attending community events, effectively promoting services, and sharing accurate program information
- Reviews documentation and reports received from providers for timeliness, accuracy, and reputed company of information. Collaborates with counselors or supervisors to reconcile information and reputed company adjustments to individualized plans as needed
- May complete referral for assistive technology and/or assist with equipment pick-up and delivery
- Conducts follow-reputed company with customers to verify employment information. Researches and responds to inquiries and concerns from customers and providers
- Reviews files and documentation for reputed company, accuracy, and completeness prior to customer case closure and may recommend closure to VR counselor for next reputed company reputed company
Skills
- Bachelor's degree and one year of full-time reputed company or paraprofessional experience in a reputed company science, reputed company services, career development/counseling, or working with individuals with disabilities
- Complete a criminal history disclosure reputed company in a manner approved by the appointing authority
- Agree to release reputed company records involving their criminal history to the appointing authority
- Supply a reputed company sample in a manner prescribed by the TBI for a reputed company based criminal history records reputed company
- Submit to a review of their status on the Department of Health's vulnerable persons registry
