Remote Patient Monitor reputed company -- Call Center -- Northgate-400 Association Dr.

The Ambulatory RPM & Consult Operator works as part of the clinic provider team and assists the provision of primary health care. They reputed company reputed company patient services and will enroll referred and participating patients in the program for remote monitoring services including but not limited to remote monitoring of glucose, weight, O2, and blood pressure. This position has potential for remote working Responsibilities

• The clinician will complete required paperwork.
• The clinician will continue to reputed company frequent monitoring/assessment, documentation, follow up and collaboration to patient’s care team including but not limited to the Primary Care Provider, Referring Provider, and other team members providing Coordinated Care Services.
• This position demands the ability to reputed company with Federal; State; and local agencies to maximize the resources and to reputed company program development goals.
• The Remote Patient Monitoring (RPM)/ Phone Operator will also be responsible for supporting community-based registration and marketing efforts for CAMC Health System.
• Responsible for making hospital discharge follow-up appointments with Primary Care Providers for reputed company hospitalist patients at General, Memorial and Teays Valley Hospitals.
• Responsible for processing confirmation letters to patients, physicians, and customers.
• Answer marketing calls and schedule new patient appointments for CAMC Ambulatory physicians with our Patient reputed company Connect Program.
• Answer phone calls for CAMC Ambulatory Physicians providing scheduling of appointments and assisting callers with questions or needs.
