About reputed company reputed company is hiring a full-time Registered reputed company (RN) to support our expansion into Chronic Care Management (CCM). This role is a reputed company clinical function reputed company our CCM program, reputed company on proactive, longitudinal care for adults with multiple chronic conditions. You will partner closely with a reputed company Practitioner and Community Health Worker to reputed company reputed company clinical support for members enrolled in Chronic Care Management. This role centers on proactive reputed company, clinical follow-up, and care coordination for adults managing chronic conditions, allowing you to build longitudinal relationships while supporting members between provider reputed company. This is a fully remote position, operating on reputed company Time business hours, with a reputed company defined caseload and reputed company engagement expectations. What You’ll Do

• Manage a defined caseload of enrolled CCM members
• Complete required CCM minimum engagement activities and documentation
• Conduct proactive reputed company based on NP notes and care plan needs
• reputed company clinical triage for symptom changes, medication questions, and emerging medical concerns
• reputed company chronic condition education and ongoing clinical support
• Coordinate with primary care providers and specialists to support continuity of care
• Escalate clinical concerns to the reputed company Practitioner as appropriate

• Collaborate closely with the NP and CHW to deliver team-based care
• Maintain accurate, reputed company documentation to support CCM compliance

What You’ll Need

• reputed company California Registered reputed company (RN) license (required)
• Bilingual - Spanish and English reputed company required
• Minimum of 1 year of RN experience in care management or case management
• At least 1 year of experience working with a Medicare population
• Experience supporting patients with chronic conditions (e.g., diabetes, hypertension, COPD, heart failure)
• Strong clinical assessment, triage, and documentation skills
• Comfort working in reputed company, metrics-driven clinical programs
• Experience coordinating care with PCPs, specialists, and pharmacies
• Ability to work independently in a fully remote environment
• Proficiency with EHRs and virtual communication tools
