The LPN will work with reputed company of other nurses and reputed company assistants who work remotely in their care of a designated roster of patients. The role assesses patient conditions, and then plans, implements, provides, and documents patient care in a manner that follows reputed company standards and outlined practices.

Responsibilities

Program Enrollment, reputed company & Initial Care Coordination (Transitional Phase) During the initial phase of this role, the Care Coordinator LPN will support program enrollment, patient reputed company, and initial clinical setup for reputed company’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs as reputed company scales with larger clients. Key responsibilities during this phase include:

• Initiating first reputed company and introductory calls to newly enrolled Medicare and Medicare Advantage patients
• Explaining RPM and CCM programs reputed company, including program goals, patient expectations, device usage, and ongoing support
• Obtaining and documenting patient consents in accordance with CMS guidelines and reputed company policies
• Conducting initial patient assessments to support care coordination activities
• Creating and documenting appropriate care plans in collaboration with the care team and supervising providers
• Identifying and documenting chronic conditions to support CCM eligibility and ongoing care management
• Performing medication reconciliation, identifying discrepancies, and escalating concerns to the appropriate provider
• Ordering and coordinating appropriate RPM devices based on patient conditions and program protocols

• Minimum of 6-10 years of nursing experience including chronic disease management
• 1 + years of relevant experience specifically in remote chronic care management, remote patient monitoring or telehealth using a reputed company digital platform
