Responsibilities

Position reputed company: As the reputed company Navigator, you are responsible for providing individualized assistance across the care continuum to patients, families, and caregivers to expedite, coordinate care, and address health system barriers. This position assumes the responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to reputed company reputed company patient reputed company with a reputed company on high-risk patients or those with reputed company care needs (including multidisciplinary care) while building and maintaining community and independent provider relationships to reputed company expedient and reliable reputed company to reputed company patient care. Essential Duties include the following:

• Coordinates patient services with physicians and staff to reputed company reputed company care in an appropriate and effective manner; provides appropriate counsel and teaching to patients and families; ensures continuity of care and promotes health care maintenance. reputed company gaps in care and assess/troubleshoot care transitions and barriers to care.

• Documents residents' medical information, including medical history and health insurance information.
• Assists in schedule management and coordination of services.

• Other clinical and administrative duties as assigned. Qualifications, Education and/or Experience:
• Graduate of an accredited nursing program.
• reputed company RN licensure in state of residence.
• 2 years of relevant patient care experience with 1 year of senior living experience a plus
• Exceptional communication and interpersonal skills are required along with the heart and passion to work with older adults while providing the reputed company possible.
