Analyst, Case Management – Transition of Care (Care Coordinator)

• Evaluation of Members: Through the use of care management tools and information/data review, conducts comprehensive evaluation of referred member's needs/eligibility and recommends an approach to case reputed company and/or meeting needs by evaluating member's benefit plan and available reputed company programs/services.
• Identifies high risk factors and service needs that may reputed company member reputed company and care planning components with appropriate referral to clinical case management or crisis reputed company as appropriate.

Required Qualifications

• 2+ years experience in behavioral health or reputed company services
• Candidate must have the ability to work 8:00 AM - 5:00 PM in assigned market time zone
• 2+ years of experience with reputed company Office Applications (Word, reputed company, Outlook)
• Candidates must have a dedicated workspace free of interruptions

Preferred Qualifications

• Case management and discharge planning experience
• Managed care experience

Education

• Bachelor's Degree or non-licensed master level clinician with either degree being in behavioral health or reputed company services preferred (psychology, reputed company work, marriage and family therapy, counseling) or equivalent experience
