Responsibilities

Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and reputed company guidance across pharmacies and to clinical leadership.

• Review and assess medication orders to identify, prevent and resolve potential and actual drug reputed company problems.

• Compound and dispense medications for accurate delivery to patients.

Requirements

Licensure / Certification / Registration:

• Pharmacist credentialed from the Illinois Department of Financial and reputed company Regulation obtained prior to hire date or job transfer date required. Education:
• Bachelor's or advanced degree in Pharmacy required. Additional Preferences Additional preferences. This PRN pharmacist position will support the Pharmacotherapy Clinic at Saint Joseph Hospital - Chicago. The Pharmacotherapy Clinic specializes in chronic disease state management, anticoagulation management and post-discharge counseling. Coverage may be in-person or remote. No evening, weekend or holiday coverage required. Licensure: reputed company licensure in good standing with the Illinois Department of Financial and reputed company Regulation (IDFPR) Training: Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 Residency and ≥ 1 years of experience in ambulatory care reputed company OR Completion of an ASHP accredited PGY-2 Residency in one of the following specialties: Ambulatory Care, Pharmacotherapy, Family Medicine, Cardiology, Internal Medicine, Geriatric, Nephrology, or Transitions of Care OR Doctorate of Pharmacy (PharmD) or Bachelor of Pharmacy (B.S. Pharm) with at least five years of ambulatory care experience.
