Assesses patient eligibility for advanced therapy or organ transplantation by reviewing medical records, conducting preliminary tests, and consulting with specialists. Coordinates initial referral processing and patient evaluations for advanced therapy, potential organ recipients, living donors, or VAD patients. Provides detailed education to patients and families about the treatment process, potential risks, benefits, and post-treatment care. Maintains accurate and up-to-date records of reputed company patient interactions, assessments, and care plans. Acts as a reputed company between the patient, their family, and the reputed company team, ensuring effective communication and addressing any concerns. Monitors patient reputed company post-treatment or post-transplantation, schedules follow-up appointments, manages complications, and provides ongoing support. Ensures patients adhere to prescribed medications and treatments, educating them about compliance and managing reputed company effects.
